Pilates For Female Athletes

Building Resilient Female Athletes: Habits, Cross-Training, and the Case for Pilates


Women's sports are having a moment, record viewership, record participation, record investment. But growth in participation has come with a harder truth that doesn't get talked about enough: female athletes are getting injured, particularly in the knee, at rates that dwarf their male counterparts, and it's happening earlier than most parents or athletes realize. The good news is that a meaningful piece of this is preventable, and cross-training methods like Pilates are becoming a serious part of how elite programs are addressing it.


The Numbers Behind the Crisis

The scale of the gender gap in injury rates is striking, and it holds up across every level of competition:

  • Female athletes face two to eight times higher ACL injury rates than male athletes, a gap driven by a mix of anatomical, hormonal, neuromuscular, and biomechanical factors.

  • In the NCAA, the rate of ACL injuries in collegiate women is three times higher than in men, and among high school athletes ages 13-18, girls tear their ACL at roughly 1.6 times the rate of boys.

  • The trend is heading the wrong direction for young female athletes specifically: ACL injury rates for high school girls rose by as much as 32% between 2007 and 2022, more than double the increase seen in young men over the same period.

  • Soccer carries the highest ACL injury rate among female team sports, at 68 cases per 100,000 athlete-exposures, and soccer and basketball together account for 70% of all female ACL injuries.

  • Girls in early-to-mid puberty, roughly ages 12 to 16, are the most vulnerable window, particularly during growth spurts, meaning the habits built in middle and high school carry real weight.

  • The consequences compound after a first injury: female athletes face four times greater risk of a second ACL injury, and six times greater risk of tearing the other knee, compared to male athletes, and also show lower return-to-sport rates and shorter careers following injury.

  • Even at the college level, competition itself is the highest-risk setting: female basketball players averaged 0.68 ACL injuries per game versus 0.10 in practice, and female soccer players averaged 1.12 in games versus 0.09 in practice, both notably higher than their male counterparts in the same sports.


This isn't a story about female athletes being "less durable." Many of these injuries are non-contact, the knee simply can't tolerate the load during a cut or a landing, even when nothing dramatic happens on the outside. That's a mechanics and preparation problem, not a toughness problem, and it's exactly the kind of problem cross-training is built to address.

Why Cross-Training Matters, Especially for Young Female Athletes

Playing one sport, year-round, with no variation in movement patterns, is one of the most consistent risk factors sports medicine researchers point to for overuse injury.


Cross-training interrupts that cycle: it gives overworked muscles and joints a chance to recover while still building strength, and it develops movement qualities: balance, control, coordination, that a single sport often doesn't train on its own.


Good habits worth instilling early, especially for high school and college athletes:

  • Train movement patterns, not just muscles. Strength without control doesn't protect a joint under a fast, unpredictable cut or landing.

  • Prioritize the posterior chain and hips. Weakness here is repeatedly linked to poor landing mechanics and higher ACL injury risk.

  • Build in recovery as part of training, not an afterthought. Overuse injuries build silently over a season; deliberate variation and rest are part of prevention, not a break from it.

  • Start early, not after the first injury. Waiting until an athlete is already hurt to introduce prevention-focused training misses the highest-value window.


Where Pilates Fits In

Pilates has moved well past its old reputation as a dancer's workout. It's become a genuine performance and injury-prevention tool used across nearly every sport, and the evidence base has grown alongside that adoption:

  • By promoting balanced muscle development, improving flexibility, and reinforcing core strength, Pilates builds a more resilient body, less susceptible to common athletic injuries like ACL tears, rotator cuff issues, or shin splints.

  • A study of experienced male runners who did mat Pilates twice weekly for 12 weeks showed a significantly faster 5K time along with reduced muscle activation on EMG testing, evidence of improved "neuromuscular economy," attributed to better control and stabilization through the lumbopelvic region.

  • Research by Çelik and Turkel (2017) found significant improvement in quadriceps strength among Pilates participants compared to control groups after just 12 weeks, directly relevant given how central quad control is to safe landing mechanics.

  • A study in basketball players found Pilates improved both flexibility and dynamic balance, both essential in a sport built around cutting, jumping, and fast lateral movement.

  • Broader systematic reviews confirm that regular Pilates practice positively affects both athletic performance and injury risk, with particular strength shown in improving core endurance, motor control, posture, and reducing recurrent injury risk.

  • Perhaps most relevant to the ACL crisis specifically: good posture, improved flexibility, muscle balance, and core stability all correlate directly with injury prevention, performance enhancement, and longevity in sport.


The common thread across this research is exactly the profile of weakness researchers keep identifying in female athletes at risk of ACL injury: poor core and hip control, muscular imbalance, and reduced neuromuscular efficiency under load. Pilates is, almost by design, built to train precisely those qualities.


A Note From My Own Work

I bring all of this into my own practice as a STOTT PILATES®-certified instructor, working with athletes at every level, including my ongoing work with the Dallas Wings of the WNBA, alongside high school and collegiate athletes building the foundation for their own careers. What I see consistently, across every level of competition, is the same pattern the research describes: the athletes with the strongest core control, hip stability, and movement awareness are the ones who stay on the court. Pilates doesn't replace strength training, speed work, or sport-specific practice, it fills in the stabilization and control layer that those methods often don't fully address, and that layer is exactly where so many of these non-contact injuries originate.


The Takeaway

The rise in ACL injuries among female athletes isn't inevitable, and it isn't a reflection of female athletes being inherently more fragile. It's a training and preparation gap, one that shows up most clearly in the mechanics of a cut, a landing, or a sudden change of direction.
